HHC's driving mission is to promote wellness, healing and comfort for patients by providing quality care at home. Our focus is on preserving a familiar environment for children and adults with complex medical issues by giving them the professional nursing, home health aide and therapy services they need in the comfort of their own homes.
By providing these services, HHC professionals facilitate the transition between hospital and home as required by the patient's medical needs, while also assisting families to adapt to new medical technologies and acting as patient advocates in the increasingly sophisticated health care environment. Services are individualized to meet every patient's specific needs, whether private duty nursing or intermittent home visits are required. An individualized plan of care includes prescribed services, treatments, and determination of expected outcomes and goals.

Quality assurance of all services provided by HHC is constantly monitored by a number of committees, including the Board of Directors, the Professional Advisory Committee, and the Quality Assurance Committee. These committees ensure that the care provided is appropriate, timely, and specific to each patient's needs. The Quality Assurance Committee also ensures that each individualized care plan meets all state and federal regulatory standards for quality care at the highest level possible. The consistent quality of services rendered by HHC is reflected by positive feedback from referral services, community representatives, and the patients and families themselves, who often stay with HHC throughout the recovery process when necessary.
HHC is also committed to providing care in a timely fashion. A simple phone call is all that is required to request services. Our operators are on call 24 hours a day, so patients and referral services can call us at any hour, day or night. Once our Home Care Coordinator completes the intake information, service can begin as early as on the same day or within 24 hours of the request.
